Using a geriatric care manager can lighten the burden and reduce stress when having to make decisions about the well being of an older adult. Geriatric care management helps with making the right decisions and finding solutions for aging safely at home, accessing the best quality care, and finding alternative senior living options.
A geriatric care manager can evaluate the level of functioning of an older adult, plan and coordinate care, monitor care, work with resistance to change, facilitate residential transitions, support adjustment, and provide information and referral for resources.
